CANCELLATION REQUEST
All cancellations must be submitted 7 days prior to the month in which the cancellation is to become effective.
Seven (7) Day Notice: Requests for changes after this time may be subject to a $50 processing fee.
If a balance is owed, please submit it. Once payment has been made your membership will be terminated. Thank you for being a member at Montecito Heights Health Club. Good luck in your future plans, and may they always include fitness and good health.
Cancellation Policy: Member may cancel or convert its membership or any portion of its membership. Member must provide management with a written cancellation or change of status form at least 7 days prior to when the cancellation or change is to take place. Members notifying Club after the 1st of the month are responsible for dues billed that month. The cancellation will not be effective until all membership cards are returned and any balance on the account is paid in full. Member may rejoin at future date by paying current initiation fee.
A member may cancel without penalty if they die, become disabled or move more than 25 miles from the facility.
This will cancel everyone on your membership. If anyone wants to remain active on the membership please complete our “Change of Membership Status” form instead.
