HEALTH QUESTIONNAIRE
Programs at Montecito Heights Health & Racquet Club are designed to enhance muscular condition, flexibility, aerobic capacity, body composition and awareness of a healthy lifestyle. There are certain risks involved with any fitness program, which include, but are not limited to: muscle soreness, joint pain, shortness of breath, overheating and in some cases, cardiovascular problems. We recommend minimizing these risks by having a physical exam before participating in our programs.
